Food Vendor Health Permit Requirements for Special Events:

Carroll County Health Department requirements have changed in 2023:

A temporary food service facility operates at a fixed location of no more than 30.

  1. Each vendor must fill out a Temporary Food License Form (below)
  2. Each vendor must include a check made out to the Carroll County Health Department for $25- and send it to Downtown Sykesville Connection with their application. (note: do not send applications or checks directly to the health department).

During a temporary food event, each vendor must supply an adequate hand-washing station, ware-washing station, ground cover, and overhead protection (see guidelines for special food service facility temporary license below). Furthermore, food must be stored and made on-site, on the day of the event.


Submitting an Application?

Payment can be submitted via the following methods:

  • Accompany your application with a check (made payable to Downtown Sykesville Connection)
  • Pay online with a credit card
  • To make an online payment, click the “Donate” button on the home page of our website.
  • All online payments are securely processed via PayPal
  • Please be sure to send a copy of your PayPal receipt to [email protected] with your application!
